My dog is still limping:
what to do if nothing is found?

The limp comes and goes, disappears during the consultation, and the first exams come back normal. This is frustrating, but it doesn't mean your dog has nothing wrong.

A limp without an immediate diagnosis isn't an imaginary limp

Your dog has been limping for several days or weeks. The limp sometimes disappears, then comes back after a walk, on waking up, or after playing. Yet during the consultation, they walk almost normally and the first exams don't show anything obvious.

This is frustrating, but it doesn't mean your dog has nothing wrong. A limp is a change in gait, most often caused by pain, discomfort or weakness. Its cause can simply be difficult to identify during a first examination.

Why can the limp be invisible during the consultation?

Some limps are constant and easy to observe. Others are much more subtle. The limp may only appear:

  • after prolonged exertion;
  • during the first steps after a rest period;
  • when the dog runs or trots;
  • on stairs;
  • when turning in a specific direction;
  • on a slippery or uneven surface.

A dog can therefore walk almost normally at the clinic while limping noticeably at home. This doesn't call the owner's observations into question β€” the conditions of the exam are simply different from those in which the discomfort usually shows up.

Filming your dog can help

When the limp is intermittent, a video taken while it's visible can provide very useful information. Try filming your dog:

  • walking, then trotting in a straight line;
  • from the front, back and side;
  • when getting up after a rest period;
  • during a U-turn;
  • before they've "warmed up".

The video doesn't replace the clinical exam, but it lets the vet observe the gait under the conditions where the problem actually shows up.

The first step is locating the pain

When a dog is limping, the priority isn't necessarily to run a CT scan or multiple X-rays right away. The first step is to try to identify the affected limb, then the region responsible for the pain. The orthopaedic exam includes:

  • observing the dog in motion;
  • palpating the muscles, bones and joints;
  • moving each joint in turn;
  • checking for instability, swelling or reduced range of motion;
  • comparing with the opposite limb.

A neurological exam may also be needed β€” certain conditions of the spine or nerves can alter gait and give the impression that the problem comes directly from a paw.

Why can a first exam be normal?

Some injuries only cause mild or intermittent pain. Others become easier to detect over time. This is notably possible with:

  • a partial ligament tear;
  • a still-subtle joint instability;
  • an intermittent kneecap (patellar) luxation;
  • a tendon or muscle injury;
  • certain elbow or shoulder conditions;
  • pain located in a toe, claw or paw pad;
  • an early condition that hasn't yet caused a visible bone change.

Muscle tension and the dog's apprehension can also make certain manipulations difficult. Light sedation sometimes allows for better muscle relaxation, more accurate joint stability testing, and correctly positioned X-rays.

Normal X-rays don't rule out every cause

X-rays are often an excellent first exam. They help assess the bones, joint alignment, osteoarthritis, certain fractures or changes linked to chronic instability. But they don't directly show every structure.

Ligaments, tendons, muscles, cartilage and some small joint lesions can be hard to assess on a standard X-ray. This doesn't mean a normal X-ray is useless β€” it already rules out several causes and helps decide the next steps.

Should you get a CT scan, ultrasound or MRI?

These exams don't all answer the same questions. Depending on the painful area and the clinical suspicion, the vet may suggest:

  • X-rays with specific views, sometimes under sedation;
  • an ultrasound, particularly useful for certain muscles and tendons;
  • a CT scan, which allows very precise analysis of bones and certain complex joints such as the elbow;
  • an MRI, especially when soft-tissue, spinal or nervous system involvement is suspected;
  • more rarely, a joint tap or arthroscopy.

The goal isn't to run every available test, but to choose the one most likely to confirm the suspicion raised during the exam.

Finding the cause doesn't necessarily mean surgery

Not every persistent limp requires surgery. Depending on the diagnosis, treatment may rely on:

  • a temporary activity adjustment;
  • pain and anti-inflammatory treatment;
  • weight loss if needed;
  • tailored rehabilitation and progressive strengthening exercises;
  • or surgery, when the instability or injury can't be corrected otherwise.

The value of an accurate diagnosis is precisely to avoid unnecessary treatments and offer a solution suited to the real cause of the problem.

In summary

If your dog is limping, there is discomfort, even if the cause isn't immediately visible. A first exam or normal X-rays don't mean the limp is imaginary or that no solution exists. Some conditions are intermittent, early-stage, or involve structures poorly visible on X-ray.

The most effective approach is to observe the limp precisely, locate the pain, then choose further tests based on that location.
